Use this service to organise your travel vaccinations and receive tailored advice for your upcoming trip abroad. To help our travel nurses assess your needs accurately, please complete the travel risk assessment form. We request you submit this form at least 6 weeks before your travel date to allow sufficient time for processing.
Once you submit the form, please allow up to 5 working days for our nursing team to review your information. After assessment, we will contact you to:
- book your vaccination appointment
- inform you of any applicable charges

Malaria Prescription Charges
Please be aware that if you require a prescription for malaria medication, there are two separate charges you will need to pay. Firstly, the surgery applies a private prescription fee for issuing the prescription. Secondly, the medication itself must be purchased at the pharmacy, where you will be charged for the cost of the medication. These charges are separate and apply individually to each person requiring the medication.
