- are registered at the surgery
Before you start
Please have the following information ready:
- your first and last name
- date of birth
- sex
- postcode
- email address
- phone number
If you are completing this form for someone else, we will also ask for:
- their details
- your relationship to them
If your name has changed
If your name has changed because of:
- marriage
- divorce
- deed poll
You will need to provide supporting evidence.
Please bring an original or copy of the relevant document to reception before we can update your records.
